For my project, I'm using the Hexagon Box Cover Dies which I designed for Honey Bee Stamps to cover their new little Hexagon Boxes.  You get a set of 5 small hexagon boxes for $3.95.

These crystal clear six-sided boxes are made from high-quality, scratch-resistant material. The material is 12 mil thick and made of durable, food-safe PET. Boxes are shipped flat. Easy assembly.  Measures: 1 3/4" x 1 3/4" x 1 1/2"

Here is a little video on making the covers with and without the cut outs.

NOTE:  When you use the cut outs, you will need to trim the flaps slightly on the lid and where the two pieces connect so they don't show through.  It's pretty obvious, you'll notice.
